• PICK UP AN APPLICATON at the Summit Home Health Care office, located at 6455 N. Union Blvd. , Suite 100 (southeast corner of Union and Dublin , in the Cottonwood Square plaza).* You may either complete the application at the office or take it with you and return it completed at a later date, or ;
• DOWNLOAD AN APPLICATON and mail it to us or bring it in. The application is available in both MicroSoft WORD format and in Adobe Reader format. To save it to your computer, right click on the link to the format you prefer and select "Save Target As..." or "Save Link As..." depending on the brower you use.
